You ask for permission for a long-term or permanent stay. You are applying for a & bdquo; green & ldquo; or & bdquo; blue & ldquo; cards You have arrived in the Czech Republic on the basis of a long-term visa granted for the purpose of obtaining a permit for long-term or permanent residence. You are requesting for the extension of the validity of the document certifying the existence of a residence permit. You are applying for a document proving that you have the right to stay, in exchange for the same certificate , which was lost, destroyed, stolen or damaged, and also instead of a document certifying the existence of a residence permit, in which the data carrier with biometric functions does not function You ask for the issuance of a new document certifying the existence of a residence permit, because the validity period has expired, or if the validity of the previous certificate is revoked (for example, because of its damage), you ask for a new document certifying the existence of a residence permit, in connection with the change of information given in it.
Biometric information collects selected & ndash; (that is, not all) & ndash; the office of the Ministry of the Interior (hereinafter only & bdquo; ministry & ldquo;), equipped with special technical equipment. In the same office where you took your biometric information, you are required to receive a document certifying the existence of a residence permit in due time. This office may be different from the office where you applied for it, or from the office where you usually solve other formal issues related to your stay in the Czech Republic.
DUTIES OF PERSONAL PARTICIPATION.
In connection with biometric information, you are obliged:
personally to appear in the office of the Ministry with the purpose of compiling a set of biometric information and providing a signature that has been determined for further digital processing under the Ministry’s summons (if you are staying in the Czech Republic) in the case of granting a long-term visa with a view to obtaining a long-term or permanent residence permit & ndash; within 3 working days from the date of entry into the territory of the Czech Republic (the representative department of the Czech Republic will grant you a long-term visa for the purpose of obtaining a permit for long-term or permanent residence) to allow and sustain the compilation of a set of its biometric data, and also to agree to the provision of a signature that is defined for further digital processing within the established timeframe *, but not later than 60 days from the provision of biometric information, personally to appear to the Ministry with the purpose of obtaining a document, the presence of a residence permit, because of the examination of your identity, or the verification of the correctness of the personal information specified in the document, as well as the functionality of the data carrier with biometric information and the correctness of the biometric information that is processed. In the event that you have left your telephone contact when applying, you will be contacted principally in this way; at the same time you will be agreed with the deadline for compiling a set of biometric information or obtaining a document certifying the existence of a residence permit. In the event that you have not left your contact, or you will not be able to contact it, you will be invited in writing to compile a biometric information package or to obtain a document certifying the existence of a residence permit. Attention: If after receiving the decision on a permanent residence permit or after issuing an order to extend the validity of this certificate issued for the purpose of confirming the right to permanent residence, you will not take the document prepared, certifying that you have the right to stay, on You will be fined, the size of which can reach 10,000 CZK. If you do not pick up a document certifying the existence of a long-term residence permit, as part of the issuance of this permit or the extension of its validity, the processing of your application will be terminated.

Certificates issued before May 2011 retain their validity until the expiration of their validity or changes in the data that they contain; after that they will be replaced by certificates with biometric information. To compile a set of biometric information for the purpose of issuing a new document certifying the existence of a residence permit, you will be summoned by the Ministry (not earlier than in May 2011).

In case of changing the data given in the certificate (for example, the surname), you are obliged to inform about the change to the ministry within 3 working days, counting from the day when the change occurred.
In the event of a change in the place of residence (residence), you are required to report the change:

For the conduct of procedures related to biometric certificates, according to the law on administrative fees, the following fees are collected (in the form of a stamp, which you can purchase at each post office):
issuance of a document certifying the existence of the right to stay: CZK 2500 (CZK 1,000) for children under 15 years old. Extension of the validity period of the holder’s card: 2500 kroons irrespective of the age of the applicant. Amendments to the document certifying the right to stay: 1000 Czech CZK issuance of a document certifying the existence of a residence permit in lieu of a similar certificate that has been damaged, destroyed, lost or stolen, and also in exchange for a document certifying the existence of a residence permit in which the data carrier with biometric information does not work: 4000 CZK (CZK 2500) – for children under 15 years old.
